Website exit survey template

Learn why visitors are leaving, whether they completed their task, and what would help them continue.

7 questions · 3 pages · 3 answer formats · About 2 min

Time is estimated from all questions, including matrix rows. Branching can shorten a path; reading, consent and longer written answers can add time. Confirm it with a pilot.

Who this is for

Web teams learning what visitors need before they leave a page or site.

Before you send it

Use a delayed or exit-intent invitation only where it will not interrupt checkout or form completion. Keep the intercept optional and brief.

Questionnaire preview

These are the questions and answer options copied into your draft. Bracketed text is a prompt to customize. Matrix questions contain several statements, so allow time for each row.

How this template adapts

Successful visitors and casual browsers skip the obstacle page. Everyone finishes with the same closing questions.

  1. Yes, completely or I was just browsing jumps from page 1 to page 3.
  2. Partly or No goes to the obstacle questions on page 2, then rejoins page 3.
  3. The two alternative answers use an ANY condition. No paid logic features are required.

Test the setup before sharing

  • Choose each of the four answers on page 1 and check the next page.
  • Compare task completion groups separately; skipped obstacle questions are not missing responses.

The preview below lists every possible question. Respondents see only the questions on their path. Open this template in the builder to test it with live answers.

How to read the results

Read task completion alongside visit purpose and next-step intent. Compare only similar pages and audiences; a single pooled exit score hides why people arrived.

Preview your edited survey on desktop and mobile, run a small pilot, and review an export before inviting the full audience. Adding questions is optional: keep the ones that support your decision.
