Employee pulse survey (eNPS) survey template
A short monthly check-in: eNPS, five statements that stay the same every round, and one question about what to change.
7 questions · 2 pages · 5 answer formats · About 1 min
Time is estimated from all questions, including matrix rows. Branching can shorten a path; reading, consent and longer written answers can add time. Confirm it with a pilot.

Who this is for
People teams and managers running a short, regular check-in.
Before you send it
Keep one link open and share it on the same day each month: the eNPS card then charts the score month by month. Small groups stay hidden, because the minimum group size starts at 5.

How to read the results
Read the eNPS beside its promoter, passive and detractor shares and the month-by-month chart, not as one number. Keep the five statements word for word every round: a reworded item starts a new series. Departments that only a few people chose are combined in filtered views; keep the list broad. These are editable items, not a validated scale.
Preview your edited survey on desktop and mobile, run a small pilot, and review an export before inviting the full audience. Adding questions is optional: keep the ones that support your decision.
