Event organizers collecting attendee needs and access preferences before an event.
Before you send it
Collect only the details needed to plan the event. Ask accessibility needs in a respectful, optional way and state who will use the information.

These are the questions and answer options copied into your draft. Bracketed text is a prompt to customize. Matrix questions contain several statements, so allow time for each row.
How this template adapts
Lunch and workshop selections reveal only the planning questions that apply to each attendee.
- Choosing Yes to lunch reveals dietary requirements on the same page.
- Selecting Workshop reveals a workshop-preparation question on the final page, even when other sessions are also selected.
- Accessibility requirements remain visible to everyone; they do not depend on session or lunch choices.
Test the setup before sharing
- Toggle lunch Yes to No and confirm dietary requirements disappear.
- Select Workshop together with another session, then remove only Workshop to verify the checkbox contains rule.
The preview below lists every possible question. Respondents see only the questions on their path. Open this template in the builder to test it with live answers.

How to read the results
Use attendance intent and session interests for planning, not as a guarantee of turnout. Keep any accommodation details private and available only to people coordinating them.
Preview your edited survey on desktop and mobile, run a small pilot, and review an export before inviting the full audience. Adding questions is optional: keep the ones that support your decision.
Keep building your study
Review event content, logistics, and future attendance intent, with room for specific improvements.
Understand the situation that started a search, competing alternatives, and the outcome a customer wanted.
Count the questions and reading load before confirming timing with a pilot.
