Usability benchmark (task-based) survey template

Compare task success and difficulty across two tasks, then capture overall usability and improvement priorities.

13 questions · 4 pages · 5 answer formats · About 3 min

Time is estimated from all questions, including matrix rows. Branching can shorten a path; reading, consent and longer written answers can add time. Confirm it with a pilot.

Who this is for

UX teams comparing the same task-based experience across designs or releases.

Before you send it

Replace Task 1 and Task 2 with testable instructions and define success before sessions. Ask each task's questions immediately afterward. Keep task wording and recruitment consistent across comparisons.

How to read the results

Compare observed task success, reported difficulty, and written friction separately. The overall battery is a custom set of items, not the System Usability Scale and should not be reported as a SUS score.

Preview your edited survey on desktop and mobile, run a small pilot, and review an export before inviting the full audience. Adding questions is optional: keep the ones that support your decision.

What you can take from this

Task 1 has 32 reported completions out of 40; Task 2 has 24. Completion includes both 'easily' and 'with difficulty'. The latter deserves attention even when the task gets done.

What to do next

Review the difficult and unsuccessful attempts before changing the flow. Run the same tasks with a comparable audience to evaluate a redesign. This questionnaire does not produce a SUS score.
