Decision study (comprehension gate) survey template

Pilot an illustrative choice task with a consent gate, scored comprehension checks, a recap branch, and debrief questions.

14 questions · 5 pages · 3 answer formats · About 3 min

Time is estimated from all questions, including matrix rows. Branching can shorten a path; reading, consent and longer written answers can add time. Confirm it with a pilot.

Who this is for

Researchers piloting a risky-choice task with scored comprehension questions and a recap branch.

Before you send it

Replace the draft consent and payout instructions, decide how a round is selected, and implement payment through your own process. Test both comprehension outcomes. The template does not calculate or send bonus payments.

Questionnaire preview

These are the questions and answer options copied into your draft. Bracketed text is a prompt to customize. Matrix questions contain several statements, so allow time for each row.

Consent gate before the questions

[Researcher: replace this draft with your study information.] Explain the purpose, tasks, expected time, compensation (if any), risks, data use, withdrawal limits, and a researcher contact. Taking part is voluntary. You may decline before starting.

Declining the gate exits without submitting a response. Replace the draft information before publishing.

How this template adapts

Scored comprehension checks decide whether a participant needs the recap before the decision task.

  1. Each correct comprehension answer earns one point. A score of 2 skips the recap and opens page 3.
  2. A score of 0 or 1 follows the recap page before joining the same task.
  3. Once the first task is answered, the score rule stops applying so participants continue to the closing questions.

Test the setup before sharing

  • Test scores 0, 1, and 2 and complete the task on each path.
  • Replace the illustrative stakes and consent text. Bonus selection and payment are handled by your own process.

The preview below lists every possible question. Respondents see only the questions on their path. Open this template in the builder to test it with live answers.

How to read the results

Keep comprehension scores with choices and report the planned treatment of recap participants. Analyze choice patterns against the specified stakes; a short illustrative task is not a validated measure of risk preference.

Preview your edited survey on desktop and mobile, run a small pilot, and review an export before inviting the full audience. Adding questions is optional: keep the ones that support your decision.
