Plan what you collect
Patients do not need to sign in, and the survey page runs no analytics or ad trackers. Do not ask for names, email addresses, or other identifying information in this use case.
Plain language, checked
The readability checker flags long wording, questions that ask two things at once, negations and vague terms. Use it to review the questions before testing them with readers.
A link per clinic or department
Use the same survey across sites. Add a clinic or department parameter to each link; it is saved with every response, so you can compare locations in the export.
You can see where people stop
Partial answers save at page boundaries. The drop-off view shows where people leave, so you can review those questions.

Design for the waiting room
Short, one question at a time, on a phone. Check the reading level, then read it aloud to five people before it goes anywhere near a clinic.

Distribute without a list
Share a QR code at reception or a link on a printed sheet. A site parameter lets you group responses without a patient mailing list.

Review the shares
Report the distribution per item, and per site from the export, rather than a single score, and quote the margin: a clinic with forty responses has a wide one.
